ALL ATTACHED PAGES TO THE STATEMENT OF VALUE WILL ALSO BE RECORDED, WITH THE EXCEPTION OF TRUSTS. EACH ADDITIONAL PAGE OVER FOUR (4) WILL BE CHARGED @$2 PER EXTRA PAGE.
