Packets of this information are available from the Office of the Treasurer of Beaver County. Be sure to bring the entire packet and all additional required information to the County Treasurers Office.
  • Completed notarized application.
  • Filled out resolution of the organization.
  • Picture of interior of the licensed premises.
  • Sketch of the premises showing exits and restrooms.
  • Copy of deed or written leasing agreement.
  • Copy of organization articles of incorporation or charter and by-laws.
  • Statement of where records are kept.

$10.00 Limited (no more than 3 in 7 days for a year)
