Information and Assistance

This is the first step in accessing Beaver County Office on Aging's Information & Referral services. An information specialist answers questions and provides resources. Information and assistance is available for obtaining a Level of Care Determination, Home and Community Based Services, APPRISE services, PACE (Pharmaceutical Assistance Contracts for the Elderly) and property tax/rent rebate. 

Information and Assistance Programs

PA MEDI – Pennsylvania Medicare Education and Decision Insight

is available to assist consumers who have questions about their health insurance, medical bills, and prescription coverage. Anyone new to Medicare or who is considering changes to insurance coverage is encouraged to call our program program for non-biased insurance counseling.

Senior Gardens

Designed to provide seniors with a gardening plot, is a spring / summer project provided by Beaver County Office on Aging. Garden plots are available to Beaver County residents over age 60. The Senior Gardens are located on County owned property in Brighton Township.

Senior Day

Senior Day at the mall is a "themed-oriented" program for seniors including vendor exhibits, entertainment, medical screenings and refreshments the second Wednesday of the month from 10 a.m. to noon, February through November. The program is held in the Beaver Valley Mall corridor in May for a Health Fair and October for a Safety, Emergency and Computer Technology Fair. The other months, the event is hosted inside Center at the Mall in the Beaver Valley Mall.

Surprises for Shut-Ins

This is an annual project that provides Christmas gifts and a friendly holiday visit to home-bound persons 60 years of age and older.

Level of Care Determination

A comprehensive assessment determines the appropriate clinical level of care for those seeking medical assistance funding for nursing home care or state subsidy for personal care home or Dom Care. Nursing home assessments also include screening for mental illness, mental retardation, and other related conditions to determine a need for specialized services. Pennsylvania Department of Human Services is responsible for determining financial eligibility.

A comprehensive assessment is completed on all new referrals, as well as reevaluations on existing BCOA participants. A care manager evaluates the person's unique needs and determines eligibility for appropriate services to meet those needs. Following the assessment, an individual care plan is developed with linkage to home-based community services.

Long Term Services and Supports through Community Health Choices

As an alternative to nursing home placement, the Department of Human Services Long Term Services and Supports through Community Health Choices provides in-home services to older persons. These participants must meet Medicaid financial requirements and be determined as Nursing Facility Clinically Eligible. The Beaver County Office on Aging is responsible for determining clinical eligibility.  Enrollment into the program is handled by the independent enrollment broker Maximus.  To get the process started call 1-877-550-4227.  The Beaver County Office on Aging is available to provide service coordination for the Aging Waiver program.

Care Management Services

Care management and community-based services are offered to seniors 60 years of age and older. A care manager is assigned to the consumer to assist with ongoing problems or concerns, as well as coordinate and arrange appropriate services. Assistance with budgeting, completing forms, linking to other community agencies, obtaining benefits, and services that may be provided.

There are two different types of community-based programs and services available:

Caregiver Support Program

This program is designed to reduce caregiver stress. The emphasis of this program is to respond directly to the needs of caregivers who are caring for a functionally dependent elderly consumer. The program also can include a grandparent or older relative, over age 55, who are a caregiver of children under the age of 18.  The grandparent or older relative must be the primary caregiver of the child(ren) and not the biological parent of the child(ren).  The caregiver and child(ren) are required to live in the same residence.  Financial reimbursement may be provided for out-of-pocket caregiving expenses, such as personal care services, supplies, assistive devices, or home modifications. Total household income determines the specific percentage of monthly reimbursement.

OPTIONS provides a variety of in-home services to consumers age 60 and older to assist in maintaining independence in the community. Services can include In-Home Meals, Adult Day Care, respite, home modifications, personal care and homemaking services.
